PRIMARY CARE SERVICES. Outlined below are specific services provided directly and under contractual agreement with the VNA of Central Jersey to Township residents.

Primary Care Services can be obtained at the VNA of Central Jersey Primary Care Center at 35 Broad Street, Keyport. Primary care is family-focused health care that emphasizes health education and wellness to promote healthy lifestyles and decrease the potential for illness. The center offers primary care services to people of all ages and is staffed by physicians and nurse practitioners. These services include health exams for people of all ages; immunizations; lead screenings; laboratory testing; tuberculosis screening and follow-up; HIV testing; illness care; schools, sports and camp physicals; referrals to community resources and specialty care; and support and self-help groups. In addition, prenatal clinics are staffed by VNA of Central Jersey nurses and Monmouth Medical Center staff. Primary Care Center hours are Monday 8:30-4:30, Thursday and Friday, 9:00-5:00; and Tuesday 11:00-7:00. For an appointment or additional information, call (732) 888-4149.
